Student ID's & Responsible Use Policy

Student ID Cards

Student's are provided their first ID free of charge.
        • Student's will be provided an ID after picture day, or make-up day. ID's are distributed in first period classes.
        • New students, or students that missed either picture day, must complete an ID Application form (see Ms. Fed).  All forms are returned to the library. If there is no picture on record, we will take one of you.
          • If you already have a picture on record from the school photographer, you will be charged $6 for the ID.
        • Replacement ID's for lost, stolen or damaged cards are charged $6 for replacement.
          • You must complete an ID Application form for replacement. Print the form by clicking on the link below, or come to the library to pick one up. All forms are returned to the library.
        • WALTER REED MIDDLE SCHOOL ASSUMES NO RESPONSIBILITY FOR LOST, STOLEN OR DAMAGED IDs.

Responsible Use Policy
Students must turn in an Responsible Use Policy (RUP) in order to use school computers, log on to the network, and use the Internet.
  • A new form must be completed each year.
  • The form is included in your emergency information packet at the beginning of the year and is to be turned in to your period one teacher.
  • RUP forms can be downloaded and printed by clicking on the links below
    • The link will open the entire District Memo.  You will only need the 3 pages that you have to initial & sign. 
    • It is available in both English and Spanish.
  • RUP forms are also available in the library in the Information Center, immediately to your right when you enter the library.
  • AFTER ID's are distributed, forms must be turned in to Ms. Fed in the library.
